Important Information About Lodging Your Licence Application
ByAt the completion of your assessment your assessor issued you with 2 copies of the Assessment
Summary, a Licence to Perform High Risk Work (LHRW) application form and within the next 14 days you
will receive a Statement of Attainment from AIMM Industrial Training.
Assessment Summary
From your assessor you were provided with 2 copies of the assessment summary as follows:
- White original copy, and
- Yellow applicant copy.
White Copy – Keep this copy in a safe place as this copy will be required at Australia Post when you
lodge your licence application.
Yellow Copy – This is your copy of the assessment summary. This copy is your interim authority to
operate a forklift when you return to work. TAKE NOTE TH IS AUTHORITY
IS ONLY VALID FOR 14 DAYS.
Ensure that you keep this copy with you at all times as it may be requested as proof of competency from
your employer or a Workplace Health and Safety QLD Inspector.
Statement of Attainment
Within 14 days of your completed assessment date you will receive a Statement of Attainment from
AIMM; this will be sent by mail to your postal address.
The Statement of Attainment is the nationally recognised certificate which outlines the competency(s)
you have attained.
When you receive the Statement of Attainment you should have it copied for your workplace records.
The Statement of Attainment will substitute the interim authority (yellow copy) and will remain in force
for 60 days.
Licence to Perform High Risk Work (LHRW) application form
This form will be provided to you by Australia Post.
Lodging your licence application
Your licence application must be lodged at any participating Australia Post Office in
Queensland. Check the Australia Post website or call 13 13 18 for details.
You should endeavor to lodge your application as soon as you receive your Statement of
Attainment from AIMM.
A time limitation of 60 days applies to all applications. If you do not lodge your application
within 60 days, your assessment result will no longer be considered as valid and you will have
to undertake the full assessment again.
When you lodge your application you will require the following:
- The completed LHRW Application form
- The original (white) copy of your Assessment Summary,
- The original copy of your Statement of Attainment, issued by AIMM.
- 100 points of Identification (check the reverse side of you yellow copy for requirements)
- A passport photo, and
- The prescribed fee – $65.00
Please note: – The reverse side of your yellow copy of the Assessment Summary also provides
a checklist and other information that may be useful when preparing your application for
lodgment.
The application must be made in person by you.
Your photographic renewable licence will be sent to your postal address by Workplace Health
and Safety QLD; this may take up to 4-6 weeks.
Whilst you wait for your photographic licence to arrive in the mail, keep your Statement of
Attainment with you at work as proof of competency.
